TDLR forms are considered government documents. As such it is considered a violation of the Texas Administrative Code (TAC) Chapter 68.12(c) for anyone to alter information on a signed TDLR form. The addition of missing information to fulfil compliance with TAC Chapter 68.12(e), without knowledge of the signer, is considered a violation of TAC Chapter 68.12(c).
TDLR understands that many projects have similar owner information and signors, but it is unacceptable to obtain a signature on a singular form and then alter the form project information to avoid signing multiple versions. If a signed form is missing information, it must be returned to the signer to complete and initial when filling in the missing information, or a new form must be completed. Offenders of alterations to a Department form may face enforcement action.
